受付に来院された際には、マイナンバーカード、または保険証をご準備のうえ、ご自身の名前と予約時間をお伝えください。また、初めての来院の場合は、簡単な登録をお願いすることがありますので、あらかじめご了承ください。
デジスマ診療をインストール済の方は、スマホのアプリを立ち上げていただき、受付にありますQRコードを読み取っていただきますと自動で受付が完了となり、便利です。
初診時は、健康保険証が必要です
